All payments including co-payments and deductibles are expected at the time services are rendered unless arrangements have been made in advance. If you have questions, we will be happy to discuss them with you. If you do not have insurance and are considered self-pay, you are expected to pay in full at the time of service. As a service to you, our office makes effort to obtain payment from your insurance carrier according to your coverage. Regardless of the type of insurance you have, you are ultimately responsible for paying your medical bills. At all times, it is your responsibility to follow up on all request from your insurance company regarding claims.

Patients with a balance of $15 or less will not receive statements. Patients with a credit of $15 or less will not be issued refund check; instead the credit will remain on the patient’s account and will be applied to future visits.

Non-Covered Services
Insurance companies often pay for services based on your diagnosis. Certain diagnoses are not payable under some insurance companies’ policies. Some examples of non-covered diagnoses are annual/complete physical exams and family/personal history of diseases. Please check with your insurance company prior to your visit.  Medoff Metabolics weight loss is a non-covered service and because of this your insurance company will not cover the cost of this service.

Surgical Procedures
Most insurance companies must be notified 5 to 10 working days in advance for non-emergency procedures. Some insurance companies consider an endoscopy, colonoscopy, hemorrhoid banding, or dilatation as surgical procedures. Contact your insurance company once your procedure has been scheduled.

Insurance – Health Maintenance Organizations (HMOs)
Some HMO plans require that you contact your primary care physician before treatment in our office. Medoff Medical is considered ‘specialist only’ on most HMO plans. It is your responsibility to bring a current referral with you at the time of treatment.

Disability Forms
Our office will not assist in the filling out/filing of disability forms.  Patients will be referred to their primary care physician for this.  These forms have become very labor-intensive, and too costly for a single physician office to complete.

If you need to cancel an appointment, we ask for a 24hour notice or we do reserve the right to charge you for your missed appointment. We also reserve the right to dismiss you form the practice if you have three visits that have been cancelled or rescheduled.
